Community Support
Rehabilitative and support service for persons with substance abuse dependence. Skilled paraprofessionals provide direct rehabilitation and support services to assist in developing services and support necessary to maintain abstinence, stable community living, prevention of a relapse and admission to higher levels of care.
Community Support is a NEPSAC program geared to empower individuals transitioning to or from treatment. Helping them to engage with the recovery community, find healthy ways to meet daily core needs, and eventually find the path to flourish in a lifetime of personal recovery.
Through consistent interaction with the Community Support Worker; the client will define and work towards individual goals.
Also, through this mutual effort, the individual sees that challenges and issues can be faced together, to discover solutions that don't always seem immediately evident. Thus, developing problem solving skill sets.
Collaborating to address common issues that underminde individuals in early recovery can include:
- Food
- Housing
- Transportation
- Employement
- Personal Resource Management (bills, budgets, etc.)
- and Social Networking (meetings, sponsorship, peers in recovery)
Eligibility Criteria

- At least 19 years of age
- Have a substance abuse or dependence diagnosis
- Live in Region I or have attended treatment at NEPSAC
- Must have completed treament or be waiting to attend treatment and need support
- Willingness to participate in community support is mandatory
- Mental and physical health must be stable and not primary
Discharge Criteria
Clients ready to discharge from SACS will:
- Be clean and sober for a minimum of six months
- Be active in a recovery support group
- Have a sponsor
- Be financially independent
- Be working towards achieving short and long term goals
